About the role:
The registered nurse II provides and coordinates care for Oscar members in a virtual patient centered medical home. You will work with a care team of Physicians, Advanced Practitioners, Nurses, Social Workers, and Medical Assistants to provide collaborative care.
You will report into the RN Manager.

Pay Transparency:
The base pay for this role is: $80,800 - $106,050 per year. You are also eligible for employee benefits, and annual vacation grant of up to 18 days per year and annual performance bonuses.
Responsibilities:
- Conduct telephonic, video or asynchronous messaging visits with patients and maintain medical records with accurate documentation in order to:
- Triage and assess new and established patients’ needs.
- Promote and support patient adherence to care planning.
- Assist with management of chronic conditions by conducting regular check-ins with patients based on acuity, and addressing care needs.
- Triage patient symptoms and requests and serve as a clinical escalation point for other care team members (Medical Assistants, LVN/LPN, etc)
- High level oversight of patients’ transitions of care such as coordinating post ER and hospital follow up needs.
- Manage pre-visit offerings by patient type and/or disease status
- Support development of expanded nursing services by providing feedback.
- Follow all member privacy and confidentiality standards of practice (i.e. HIPAA)
- Compliance with all applicable laws and regulations
- Other duties as assigned
Requirements:
- Bachelors of Science in Nursing Degree or Associate of Science in Nursing from an accredited School of Nursing
- Active, unrestricted Licensed Registered Nurse (RN) in the states of TX, GA, FL or a compact license covering all 3 states, with no disciplinary action
- 3+ years of outpatient / ambulatory RN experience
- 1+ years of working knowledge of Milliman Care Guidelines
Bonus points:
- 1+ years experience in basic behavioral health interventions used in primary care
- 1+ years experience in virtual based practice
- Knowledge and/or familiarity with the principles of project and program management
- Willingness to obtain additional state licenses as needed
- Spanish language fluency
