Description and Requirements
The Registered Dietitian, Care Manager is responsible for assessing the nutritional needs of members and helping them achieve an optimal level of independent functioning. The Registered Dietitian, Care Manager collaborates with a care management team (which may include RN's, Social Workers, and Coordinators) to evaluate members and provides education and consultation to members, family/caregivers, and other people responsible for the member's shared care.
- Consults with and educates members, families/caregivers, and other relevant care team members on nutritional needs and pertinent care plans. - Ensures compliance with applicable NYS regulations. - Documents member and provider interactions in compliance with our internal policies. - Collaborates with the care management team as needed to safeguard the appropriateness and effectiveness of services recommended/utilized. - Additional duties as assigned.

Hiring Range:
- Greater New York City Area (NY, NJ, CT residents): $67,205 - $95,680
- All Other Locations (within approved locations): $59,800 - $87,360
As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
